WHAT TO BRING TO MY ASSISTANCE APPOINTMENT

Do you have an appointment for help at Good Samaritan Ministries and can’t remember what to bring? Here is a quick reminder of all the documentation you will need.

Please make sure you bring all the required documents, otherwise, we cannot assist you!

Financial Assistance Required Documentation

  • Rental assistance: eviction notice, rental and/or lease agreement, photo ID, and Social Security Card. We cannot help with deposits or first-month rent.
  • Auto Repair: an estimate from a car repair shop, driver’s license, registration, proof of insurance (must be in your name), photo ID, and Social Security Card.
  • Propane: quote from propane provider, photo ID, and Social Security Card.
  • Medical Prescription: prescription, estimate, photo ID, and Social Security Card.
  • Gas (only for Medical Appointments): proof of medical appointment, insurance, registration, driver’s license, photo ID, and Social Security Card.
  • Utilities: photo ID, Social Security Card, current bill, shut off notice, proof of energy share, or LIEAP that applies, and documentation if assistance has been provided or not. Proof of any appointments with NWE/LIEAP. If the individual asking for assistance has broken their payment plan; Good Samaritan Ministries will not be able to assist.

In-Store Assistance Required Documentation

  • Diapers/formula: parent Social Security Card, Identification Card/driver’s license, children’s Social Security Cards. Individuals are allowed five (5) visits for diapers and/or formula per year.
  • Clothing: Social Security Card, Identification Card/driver’s license for all who are getting assistance
  • Household items: Security Card, Identification Card/driver’s license
  • Furniture: truck to transport furniture, Security Card, Identification Card/driver’s license.
